Checklist of Documents Needed for SBA Loans

Here are the documents you will need for to process your SBA Small Business Loan:
1. A completed and signed “Application for Business Loan” (SBA Form 4)
2. A completed and signed “Statement of Personal History” (SBA Form 912) for each individual that has 20% or more ownership and/or each partner
3. Copies of the last three years of Federal Income Tax Returns, including all schedules
4. Copy of most recent Federal Income Tax Return, including all schedules, for each affiliate
5. A current Income Statement within the past ninety-days (90) of the application submission (Profit/Loss)
6. A current Balance Sheet within the past ninety-days (90) of the application submission (Balance Sheet)
7. Most recent personal Federal Income Tax Return for each individual that has 20% or more ownership and/or each partner
8. A completed and signed “Personal Financial Statement” (SBA Form 413) for each individual that has 20% or more ownership and/or each partner
9. A Tax Information Authorization form (IRS Form 8821) completed and signed by an authorized individual for the business, for each affiliate business and for each individual with ownership interest
10. Documentation to establish ownership, including: Articles of Incorporation, Partnership Agreement, Corporate Resolution, By-Laws, and Assumed Name Certificate
11. List of Assets and Debts of the company
12. Business Plan with minimum of three years of projections for Income Statement, Balance Sheet, Cash Flow, Executive Summary and Narrative
13. Management Summary
14. List of Collateral
15. Listing and aging of Accounts Receivable and Accounts Payable
16. Names and addresses of at least three suppliers
17. Disclosures and narrative relating to prior bankruptcies
